Port Huron ENT, PC Closure Notice
Dear Patients of Port Huron ENT,
This is to inform you that the medical office of Port Huron ENT, PC will be closing on June 30, 2025 due to Dr. Obermyer’s retirement. It has been an honor and privilege to have been your health care provider. We are grateful for the opportunity to have served your medical needs. We will no longer be available to take care of your medical needs after that date.
It is important that you continue with appropriate medical care, therefore you should establish contact with another physician as soon as possible. We will do what we can to help facilitate your transition of care, but it will be up to you to take steps to connect with a provider of your liking.
If you are not acquainted with another physician, you may use your insurance company’s provider directory, check with your primary care physician for a referral, or check with the hospital referral service.

Your health records have been transferred to Morgan Records Management, LLC. They will continue to maintain your records in accordance with applicable confidentiality and security standards and with applicable laws. Your records will be destroyed no less than 10 years after the last date of services you received from this Port Huron ENT.
If you wish to have a copy of your records sent to you or to another provider, you may submit a request to the records custodian, as follows:

Allergy patient records will be available through our office through June 30, 2025 and then will be at the records custodian, Morgan Records Management.
Please note: In accordance with applicable state law, you may be charged for the copying and transmittal of the records to you or to another healthcare provider. There may be a period of time after the records have been transferred before they can be accessed.
